Colonial Medical Center is now offering Televisit services for Pediatric and Adult Patients. Televisit services are available for patients needing medical care.
To schedule a Televisit appointment with Dr Eduardo Correa, Dr Sandra Pena and Dr Natverlal Surati, please call to our office at 773-248-8644 and request a Televisit.
Or, you can schedule your appointment via Zocdoc or Healow (links are found below) and include a request for a Televisit option in the appointment details and you will be notified within 24 hours.

You can book a Televisit appointment for follow-up visits and new patient visits. Please note this is a billable service. If this is not covered by your Healthcare Insurance, you will be responsible for covering costs ($75.00)

What is Healow?
Healow is a platform which allows patients and providers to stay engaged and to help patients take charge of their health.
Ensure you are enabled for our Patient Portal and know your username and password when your appointment is booked.
How Do I Access Healow?
New Patients
For new patients that do not have Healow account, you have to contact our office to register. If you call after working hours, you can leave a message and we will call you during business hours.
Established Patients
If you have an active patient portal account with username and password you can schedule a Televisit appointment (include a request for a Televisit option in the appointment details) or you can call to our office. If you call after working hours, you can leave a message and we will call you within 24 hours.
